For one flat monthly membership fee, you receive unlimited office visits and telehealth visits with our primary care team. No copays. No per-visit charges. Just straightforward access to the care you need.

What’s Included?

Your membership includes unlimited visits for your primary care needs, including:

• Comprehensive physical exams
• Routine and problem-focused office visits
• Telehealth visits
• Cryotherapy
• B12 injections
• Joint injections
• Trigger point injections
• EKGs, including the report
• Urinalysis
• Glucose testing
• Nebulizer treatments

Because your membership covers care that can be provided directly in our office, you can address many common health concerns without worrying about an additional office-visit charge.

What Isn’t Included?

Your monthly membership covers the services we provide in our office. It does not include outside services or testing that cannot be performed in our office, including:

• Laboratory testing
• Imaging such as X-rays, CT scans, MRIs, or ultrasounds
• Services provided by specialists
• Hospital or emergency care
• Other services performed outside our practice

Although laboratory testing is not included in the membership, our patients receive discounted pricing at Quest Diagnostics locations, helping make necessary lab work more affordable.
